Annapurna Base Camp Insurance: Complete Guide
Suhana Shrestha
Annapurna Base Camp sits at 4,130 meters (13,550 feet), inside a natural amphitheater of peaks over 7,000 meters, at the end of one of Nepal's most popular trekking routes. Getting there safely depends on more than fitness and good boots. It also depends on having the right insurance, coverage that actually pays for a helicopter if something goes wrong at altitude, days from the nearest road.
This guide explains what Annapurna Base Camp (ABC) insurance is, whether you need it, what a policy should include, what it typically costs, and how to avoid the coverage gaps that catch trekkers out every season.
What Is Annapurna Base Camp Insurance?
Annapurna Base Camp insurance is not a separate product sold under that name. It's ordinary travel or trekking insurance that has been checked, or specifically purchased, to cover the conditions of the ABC trek: an altitude ceiling above 4,130 meters, emergency medical treatment, and helicopter evacuation from a remote mountain trail.
Many standard travel insurance policies exclude "hazardous activities" above a set altitude, often 3,000–4,000 meters, unless you add an upgrade. A policy that works fine for a beach holiday may quietly stop covering you exactly where you need it most on the ABC trail.
Do You Need Insurance for the Annapurna Base Camp Trek?
Short answer: Yes, in practice, even though the legal picture is not perfectly uniform. No reputable trekking agency in the Annapurna region will guide you above roughly 3,500 meters without proof of valid insurance, and the Annapurna Conservation Area Permit (ACAP) you're required to carry does not include any medical or rescue cover of its own.
Whether insurance is a nationwide legal requirement for every foreign trekker is genuinely inconsistent across current sources: some report it is now expected for all foreign trekkers, while others describe it as a hard legal requirement only for restricted-area permits, Manaslu, Upper Mustang, Tsum, and Nar Phu since March 2026, with the Annapurna Conservation Area treated differently because it isn't a restricted area. What's consistent everywhere is that agencies, guides, and rescue coordinators treat insurance as non-negotiable in practice. Confirm the current rule directly with your trekking agency or the Nepal Tourism Board before you travel, since this is exactly the kind of requirement that gets updated between seasons.
Permits, Guides, and Where Insurance Fits In
Two other requirements sit alongside insurance for the ABC trek:
ACAP (Annapurna Conservation Area Permit): required for every trekker entering the Annapurna Conservation Area, currently NPR 3,000 (about USD 22–23) for foreign nationals, checked at multiple checkpoints including Birethanti and Chhomrong.
A licensed guide: mandatory for all foreign trekkers in the Annapurna region since April 2023. Independent, unguided trekking is no longer permitted on this route.
The status of the TIMS (Trekkers' Information Management System) card is one of the most inconsistently reported details in current trekking guidance, some sources describe it as discontinued for Annapurna routes, others say it's still issued alongside ACAP but rarely checked. Ask your agency to confirm which permits they arrange for you, and don't assume last year's rule still applies.
None of these permits or requirements are a substitute for insurance. ACAP funds conservation work; TIMS (where still used) tracks trekker movements; your guide manages the trek day to day. Only an insurance policy pays the bill if you need a helicopter.
If you're arranging insurance locally rather than relying solely on a policy from home, Himalayan Guardian Nepal's CTG (Comprehensive Tourism Guard) Trekking Package is built for exactly this altitude range, it covers routes up to 5,500 meters, which comfortably includes Annapurna Base Camp's 4,130m, and pairs the policy with a GPS-enabled tracking device rather than paperwork alone. It's worth treating as a complement to, not a replacement for, any travel insurance you already carry from home, since it's specifically designed for the ground-level rescue-coordination layer in Nepal.
At minimum, a policy suitable for the ABC trek should include:
- Emergency medical treatment at a hospital in Pokhara or Kathmandu
- Helicopter evacuation explicitly rated for your trek's maximum altitude (4,130m+ for ABC, with a safety margin)
- Emergency medical evacuation and repatriation back to your home country if needed
- Trip cancellation or interruption, covering non-refundable costs if illness, injury, or an emergency cuts the trek short
- Loss, theft, or delay of baggage and gear, useful for expensive trekking equipment
- 24-hour emergency assistance line, ideally one that can coordinate directly with helicopter operators and hospitals rather than requiring you to pay and claim later
A policy that covers all of these but caps the altitude limit at 3,500m or 4,000m is not adequate for ABC, the base camp itself sits above that line, and several days of the standard route (Deurali, Machapuchare Base Camp) are above 3,700m as well.
Helicopter Evacuation Coverage
This is the single most important line item in any Annapurna Base Camp insurance policy. There are no roads to Annapurna Base Camp. If someone develops severe altitude sickness, breaks an ankle, or has a medical emergency near Machapuchare Base Camp or ABC itself, a helicopter is often the only way to reach a hospital in hours instead of days.
Confirm three things specifically:
- The policy's altitude limit is stated in the wording (not just "high altitude trekking") and covers at least 4,500m
- Helicopter evacuation is included, not just "emergency medical transport," which can sometimes mean ground ambulance only
- The insurer either pays the helicopter operator directly (a "guarantee of payment") or reimburses quickly, Nepali helicopter companies frequently ask for payment or written insurance confirmation before they will fly
What Helicopter Rescue Actually Costs
There is no single official published rate for helicopter rescue in Nepal, pricing depends on the operator, distance, altitude, and weather. Multiple current trekking-industry sources converge on a broad range: a rescue from the Annapurna or Khumbu region typically runs from around USD 3,000 to USD 10,000, with some sources citing figures as high as USD 12,000–15,000 for rescues from higher or more remote points, before hospital treatment or repatriation are added. Treat any specific figure quoted to you, including the ranges in this guide, as an estimate rather than a guaranteed price, always confirm current rates with your insurer or agency before you trek.
Without insurance, this cost falls on you, and Nepali helicopter operators generally require payment guarantee, cash, or a credit card before departure not always after the flight.
A Word of Caution About Unnecessary Evacuations
Industry reporting has documented isolated cases of evacuations arranged for trekkers with only mild symptoms, sometimes tied to referral incentives for the guide involved. This is not the ordinary experience of trekking in Nepal, where most evacuations are genuine and appropriately called but it's worth booking with an established, TAAN-registered agency, and asking how evacuation decisions are made before you go.
This is also part of why device-supported insurance has started to gain traction in Nepal. Himalayan Guardian Nepal's CTG system issues each insured trekker a GPS tracking device that logs altitude, route, and movement, so that a rescue claim can be checked against real location data rather than a phone call alone. It doesn't eliminate the need for good judgment on the trail, but it's a practical, verifiable answer to a real and openly discussed problem in the industry.
Altitude Sickness and Medical Coverage
Altitude sickness on Nepal treks is the most common reason trekkers need medical attention or evacuation on the ABC trek. According to the U.S. Centers for Disease Control and Prevention (CDC), altitude illness becomes a risk above roughly 2,500 meters, and its most common form, acute mountain sickness (AMS), can affect a large share of people who ascend quickly without proper acclimatization.
There are three recognized stages of altitude illness:
- Acute Mountain Sickness (AMS): headache, nausea, fatigue, and dizziness. Usually resolves with rest and stopping ascent.
- High-Altitude Pulmonary Edema (HAPE): fluid in the lungs, causing breathlessness and cough. A medical emergency requiring immediate descent.
- High-Altitude Cerebral Edema (HACE): brain swelling, causing confusion, loss of coordination, and severe headache. The most dangerous form, and also a medical emergency requiring immediate descent and evacuation.
Both HAPE and HACE can become life-threatening within hours if untreated, which is why insurance that includes rapid helicopter evacuation, not just hospital treatment after the fact, matters so much on this route. The CDC and most trekking-medicine guidance recommend gradual ascent, staying hydrated, and not pushing on if symptoms worsen, no insurance policy replaces sensible acclimatization practice, and most exclude claims where a trekker knowingly ignored medical advice to descend.
Emergency Rescue: How It Works on the Trail
- Symptoms are recognized by the trekker, guide, or teahouse owner, and the decision is made to stop ascending or descend.
- The guide contacts the trekking agency's office, typically by phone where there's signal.
- The agency contacts the insurer's emergency line to confirm coverage and, where possible, secure payment guarantee for the helicopter operator.
- A helicopter is dispatched, weather permitting. Flights generally run daylight hours only, and mountain weather can delay a rescue.
- The patient is flown to Pokhara or Kathmandu for treatment, or to lower altitude to recover before rejoining the trek.
This is also why a 24-hour assistance line and pre-authorized payment matter more in Nepal than they might on a lower-stakes trip: waiting to sort out reimbursement after the fact can delay a flight that needs to happen quickly.
What Annapurna Base Camp Insurance Usually Does Not Cover
Policies vary, but common exclusions worth checking for include:
- Trekking above the policy's stated altitude limit (a frequent, costly gap for ABC specifically)
- Pre-existing medical conditions not disclosed at purchase
- Injuries connected to alcohol or drug use
- Trekking without a licensed guide, on some policies, given the current guide requirement
- Search-and-rescue costs when a person is reported missing but not confirmed to need medical evacuation (sometimes covered separately, sometimes not at all)
- Claims arising from ignoring a guide's or doctor's advice to descend
Always read the policy wording directly rather than relying on a summary page. Exclusions are where general marketing claims and the actual contract can diverge.
How Much Does Annapurna Base Camp Insurance Cost?
Trekking and travel insurance pricing depends on your age, trip length, home country, and the coverage limits you choose, so there is no single reliable figure that applies to every trekker. As a general pattern reported across current trekking-insurance guides, a policy with adequate high-altitude and helicopter evacuation coverage for a 1–2 week Nepal trek tends to run from roughly USD 40 to over USD 150, though this varies significantly by provider and country of residence. Treat any quoted price as a starting point for comparison, not a guarantee, and get a quote from the insurer directly before booking your trek.
How to Choose the Right Insurance Policy
- Start with the altitude limit. If it doesn't explicitly state coverage above 4,130m (ideally with margin, since some itineraries add side trips), move on.
- Confirm helicopter evacuation is named, not implied. Some policies cover "emergency transport" that turns out to mean ground transport only.
- Check how claims are paid. Direct payment or guarantee-of-payment to the hospital and helicopter operator is far less stressful than out-of-pocket payment followed by reimbursement.
- Look for a 24-hour multilingual assistance line that has experience with Nepal specifically.
- Read the exclusions section in full, not just the coverage highlights.
- Compare trip cancellation and gear coverage, especially if you've paid for a guided package in advance.
What to Check in Your Policy Before You Buy
| Item | Why Check It? |
|---|---|
| Stated altitude limit | Most standard policies stop well below Annapurna Base Camp's 4,130m |
| Helicopter evacuation, named explicitly | "Emergency transport" can sometimes mean ground transport only |
| Direct payment to hospitals/helicopter operators | Avoids needing to front thousands of dollars during an emergency |
| Pre-existing condition disclosure rules | Undisclosed conditions are a common reason claims are denied |
| Guide requirement clauses | Some policies require a licensed guide to remain valid, matching Nepal's current rules |
| 24-hour assistance line, tested before departure | A number that doesn't answer at 2 a.m. in the mountains is not useful |
What to Do During an Emergency
- Stay calm and stop ascending immediately if you or a companion shows symptoms of AMS, HAPE, or HACE.
- Alert your guide or the nearest teahouse owner right away, don't wait to see if symptoms pass on their own.
- Call your insurer's 24-hour emergency number, which should be saved in your phone and written on paper as a backup.
- Begin descent if instructed, even a few hundred meters can meaningfully improve altitude symptoms.
- Keep your policy number, passport, and ACAP permit accessible, rescue coordination moves faster with documents ready.
- Get receipts for anything you pay for, including medication, transport, or hospital deposits, for reimbursement later.
Insurance Documents and Information to Carry
- A printed copy of your policy, including the 24-hour assistance phone number
- Your policy number and insurer's claims email address
- Passport and a photocopy, kept separately from the original
- ACAP permit (and TIMS card, if your agency issues one)
- Emergency contact details for someone at home
- A basic note of any pre-existing conditions and current medications, in case you're unable to communicate them yourself
Common Mistakes Trekkers Make
- Assuming a credit card's built-in travel insurance covers high-altitude helicopter evacuation. Most don't, or cap altitude well below 4,130m.
- Buying a generic travel policy without checking the altitude clause, the most common gap for ABC trekkers.
- Not disclosing pre-existing conditions, which can void a claim entirely.
- Continuing to ascend despite worsening symptoms, often due to summit pressure or a fixed itinerary.
- Not saving the emergency assistance number offline, where mobile signal is unreliable.
- Assuming ACAP or TIMS includes medical coverage. Neither does

Frequently Asked Questions
Do I need travel insurance for the Annapurna Base Camp trek? Yes, in practice. While the exact legal requirement varies by source and by permit type, no established trekking agency will guide you above roughly 3,500m without proof of valid insurance, and ACAP does not include medical or rescue coverage.
Is Annapurna Base Camp trek dangerous? It is a moderate-difficulty trek that most reasonably fit people can complete, but it does reach 4,130m, where altitude sickness becomes a real risk. The main dangers are altitude illness, slips on steep or icy sections, and, rarely, extreme weather. Sensible acclimatization and proper insurance significantly reduce the risk.
Does travel insurance cover helicopter evacuation in Nepal? Only if the policy explicitly says so. Many standard travel insurance policies exclude high-altitude trekking or cap coverage below 4,000m. Confirm the altitude limit and that "helicopter evacuation" is named in the policy wording, not just implied under general emergency transport.
How much does a helicopter rescue cost in Nepal? Current trekking-industry sources put the typical cost at roughly USD 3,000–10,000, with some citing up to USD 12,000–15,000 for remote or high-altitude rescues. Costs vary by operator, distance, and weather, so treat any figure as an estimate and confirm current pricing with your insurer.
What altitude should my insurance cover for Annapurna Base Camp? At least 4,130m, ideally with a margin above that, since some itineraries include side trips or detours that add elevation.
Does insurance cover altitude sickness? Most trekking insurance policies cover medical treatment and evacuation related to altitude sickness (AMS, HAPE, HACE), as long as the altitude limit in the policy covers where the illness occurred. Coverage can be denied if you trekked above your stated altitude limit or ignored medical advice to descend.
What does Annapurna Base Camp insurance usually not cover? Common exclusions include trekking above the policy's altitude limit, undisclosed pre-existing conditions, alcohol- or drug-related injuries, and situations where the trekker ignored a guide's or doctor's advice to descend. Always read the exclusions section directly.
How much does Annapurna Base Camp insurance cost? Pricing depends on age, trip length, home country, and coverage limits. Reported ranges in current trekking-insurance guides generally fall between roughly USD 40 and USD 150+ for a one- to two-week Nepal trek with adequate high-altitude cover, though this varies by provider.
Do I need a permit for Annapurna Base Camp? Yes. You need the Annapurna Conservation Area Permit (ACAP), currently NPR 3,000 for foreign nationals, and you're required to trek with a licensed guide as of the rules in force since April 2023. The status of the TIMS card is inconsistently reported across current sources, so confirm with your agency.
Can I trek Annapurna Base Camp without a guide? No, not currently. A licensed guide has been mandatory for foreign trekkers in the Annapurna region since April 2023, and independent solo trekking is no longer permitted on this route.
What is the death rate on the Annapurna Base Camp trek? There is no single, regularly updated official statistic specific to Annapurna Base Camp that this guide can responsibly cite. Fatalities on the route are uncommon relative to the number of trekkers, and most serious incidents relate to altitude illness, falls, or, rarely, weather. Proper acclimatization, a licensed guide, and adequate insurance address the majority of the risk.
What should I do if I get altitude sickness during the trek? Stop ascending immediately, tell your guide, and descend if symptoms don't improve with rest. Contact your insurer's emergency line if symptoms are moderate to severe. Do not continue ascending while symptomatic.
Is regular travel insurance enough for Annapurna Base Camp? Usually not, unless you've specifically checked that it covers high-altitude trekking and helicopter evacuation above 4,130m. Many standard policies exclude "hazardous activities" or cap altitude well below this.
Annapurna Base Camp is a genuinely achievable trek for most reasonably prepared hikers, and it draws tens of thousands of trekkers a season for good reason. The insurance side of planning deserves the same attention as fitness training and gear. A policy that explicitly covers helicopter evacuation above 4,130m, includes real medical and repatriation coverage, and offers a responsive 24-hour assistance line is the difference between a costly inconvenience and a genuine financial and medical safety net if something goes wrong on the trail.
Before you leave home, check your existing travel insurance against the checklist above altitude limit, named helicopter evacuation, and a responsive assistance line. If you want a Nepal-based layer on top of that, Himalayan Guardian Nepal's CTG Trekking Package is built specifically for routes like this one, combining altitude-appropriate coverage with a GPS tracking device so a rescue team can find you quickly and confirm the emergency is real.
