Manaslu Circuit Trek Insurance: Altitude, Rescue & Coverage Guide (2026)
Suhana Shrestha
Trail status, 29 September 2026. A flash flood on the Therang Khola near Namrung on 6 September destroyed two bridges. The Gorkha District Administration Office asked trekkers to avoid the Manaslu and Larke Pass routes until further notice. One operator reports that access resumed after a temporary bridge and alternative trail were set up. We could not confirm a formal reopening notice. The UK government also warns of flood and landslide risk across Gorkha after the 26 August floods. Confirm current status with your agency, and ask your insurer how it treats travel during official closures or advisories.
Quick Answer: What Insurance Should You Have for the Manaslu Circuit?
| Layer | What can be verified |
|---|---|
| Nationwide law | HGN's review found no Nepal Tourism Board or Department of Immigration publication saying every trekker must hold insurance. Websites citing an April 2025 rule with fixed evacuation minimums could not be traced to an official notice. Other operators state the opposite: proof of insurance mandatory for restricted-area permits since March 2026. If someone quotes a rule, ask for the notice. |
| Restricted-area permit file | The Department of Immigration lists documents relating to the insurance of foreign trekkers and accompanying Nepali staff, and requires personal accident insurance for Nepali guides and porters. It publishes no minimum cover or altitude. |
| Entry and operator | The UK government says border officers may ask to see a travel insurance document covering healthcare and repatriation. That is its reading of Nepal's rules for British passport holders. Agencies set their own conditions. |
| Recommendation | The UK government advises making sure your insurance includes mountain rescue services and helicopter costs. |
The restricted-area permit process itself (agency, licensed guide, permit fees) belongs in the Manaslu itinerary planning guide. For the wider picture, see Nepal trekking insurance requirements.
Why Manaslu Requires Different Insurance Considerations- Remoteness. Once past Jagat there is no easy exit. HGN's route guide describes 14-plus consecutive days of trekking with limited evacuation access between Namrung and Bimthang.
- Altitude gain. The standard route climbs from Samagaun to Samdo to Dharamsala, then crosses the pass. That is several nights above 3,500 m before the highest day.
- Medical access. Villages have basic health posts. Advanced treatment is in Kathmandu.
- Communications. Guides may rely on satellite phones or devices. Delayed contact means delayed assessment.
- Evacuation complexity. The UK government warns that rapid helicopter evacuation may be difficult in remote areas. Operator estimates for dispatch time vary widely. One says 30 to 60 minutes from Samagaun or Samdo in clear weather. Another says 2 to 5 hours from decision to hospital. Treat these as illustrations, not promises.
What Is the Highest Altitude on the Manaslu Circuit?
| Point | Approx. altitude | Note |
|---|---|---|
| Samagaun | ~3,530 m | Usual acclimatisation stop |
| Samdo | ~3,860 m | Cell |
| Dharamsala (Larkya Phedi) | ~4,460 m | Overnight before the crossing |
| Larkya La | ~5,106 m (sources: up to 5,160 m) | Highest point of the standard circuit |
| Bimthang | ~3,590–3,720 m | First stop after the pass |
| Manaslu Base Camp (optional) | ~4,800–4,900 m | Side trip; sources vary |
Approximate; figures vary by source by tens of metres. Route figures follow HGN's itinerary table and operator data.
- Write down your highest sleeping altitude and your highest day altitude.
- Add optional side trips such as Manaslu Base Camp.
- Use the highest published figure for Larkya La.
- Pick a policy band above that figure.
- Read the definition of "trekking" in the policy.
What Should Manaslu Trek Insurance Cover?
| Feature | Why it matters on Manaslu | What to verify |
|---|---|---|
| High-altitude trekking | The pass exceeds 5,000 m | Altitude band vs. Larkya La; "trekking" defined |
| Accidental medical treatment | Injuries on steep, rocky trail | Limit, deductible, treatment in Nepal |
| Altitude-related illness | AMS, HAPE, HACE risk rises with height | Named wording; whether treatment and evacuation are covered |
| Emergency medical transportation | Moving a patient to care is the core benefit | Methods covered; limit; who authorises |
| Helicopter evacuation | Often the fastest option when feasible | Medical-necessity test; excess; feasibility conditions |
| Ground evacuation / ambulance | May be the only or safer option | Explicitly included, not only air |
| Search and rescue | Locating someone can itself be costly | Whether excluded or sub-limited |
| Repatriation | Medical return or return of remains | Limits; what triggers it |
| Trip interruption / delay | Weather or trail closures can disrupt plans | Whether closures and advisories are covered |
| Guides and porters | They are not automatically insured under your policy | CTG excludes guides, porters and Sherpas from the insured class; agencies handle their cover separately |
Does Manaslu Trek Insurance Cover AMS, HAPE and HACE?
- AMS (acute mountain sickness). Headache is the cardinal symptom, usually with loss of appetite, dizziness, fatigue or nausea. Onset is typically 2–12 hours after ascent, and it generally resolves within 12–48 hours if you don't climb higher.
- HACE (high-altitude cerebral edema). Considered "end-stage" AMS, with neurological signs such as confusion, poor coordination and drowsiness. Coma can follow within 12–24 hours of ataxia without treatment or descen
- HAPE (high-altitude pulmonary edema). Starts with chest congestion, cough and unusual breathlessness on exertion, and can progress to breathlessness at rest. Descent is urgent and usually mandatory.
- Medical necessity. A guide, medic or doctor judges that the patient needs to be moved.
- Assessment and authorisation. The insurer or its assistance partner verifies the case and location, and checks the policy.
- Weather and terrain. Cloud, wind, snow and landing sites are assessed.
- Operational feasibility. An aircraft and crew must be available and able to fly safely.
- Evacuation method. Air, ground or a combination, whichever the situation allows.
| Factor | Helicopter | Ground / descent |
|---|---|---|
| Speed | Fastest when feasible | Slower, but may start immediately |
| Weather dependence | High: flies under Visual Flight Rules | Lower |
| Terrain | Needs a viable landing site | Needs a passable trail |
| Altitude effects | Thin air reduces lift and engine power; payload is restricted | None |
| Medical role | Moves patient to hospital | Descent is the definitive treatment for AMS, HAPE and HACE |
| Availability | Aircraft, crew, daylight | Guide, porters, walkable route |
When descent is impossible, oxygen or a portable hyperbaric bag, with drugs, can be lifesaving. The decision belongs to the guide, medical advisers and the coordination team, not to the patient's preference alone. Why Standard Travel Insurance May Not Be Enough
- Altitude limits. A cap below about 5,200 m does not clear Larkya La.
- Activity definition. "Trekking", "hiking" and "mountaineering" are often defined separately.
- Adventure-activity exclusions. Some policies exclude hazardous activities outright.
- Evacuation limits. Look at per-claim caps, excesses and whether search and rescue is included.
- Policy wording. Rely on the schedule and wording, not marketing pages. Cover and exclusions can differ by your country of residence.
Common Manaslu Trek Insurance Exclusions to Check
- [ ] Altitude ceiling below your highest point, including side trips
- [ ] Trekking not defined, or defined below your route's altitude
- [ ] Search and rescue excluded or sub-limited
- [ ] Pre-existing conditions and disclosure requirements
- [ ] Alcohol or intoxication clauses
- [ ] Travel against official advisories or closures (relevant in the current flood period)
- [ ] Non-permitted routes. CTG requires legally permitted routes
- [ ] Pre-authorisation. Self-arranged evacuation without contacting the insurer can complicate a claim. CTG allows a limited exception where prompt contact wasn't possible, subject to verification
- [ ] Notification and documentation deadlines. CTG: notify within 48 hours and submit documents within 21 days
- [ ] Helicopter excess or sub-limits
- [ ] Age limits. CTG: 18–70
- [ ] Start date and waiting periods
- [ ] Mountaineering and peak climbing excluded
What Happens During an Emergency on the Manaslu Circuit?
- SOS or emergency report. Your guide or you contact the emergency line or activate a device.
- Assessment and verification. Location, symptoms and urgency are established. Readings and photos help.
- Coordination. The insurer, assistance partner, agency and rescue provider align on the plan and cover.
- Ground or air response. Chosen by condition, weather, terrain and feasibility.
- Medical facility. Handover and treatment, then documentation for any claim.
- Does the policy cover trekking to at least 5,200 m on this route?
- Is the band 5,500 m or higher, and does it apply to trekking?
- Are AMS, HAPE and HACE named or clearly covered?
- Is emergency medical transportation covered, and which methods (air, ground, ambulance)?
- Are search and rescue costs covered?
- What are the limits, excesses and sub-limits?
- Does the insurer pay providers directly, or do you pay and claim?
- Who must authorise evacuation, and how do you reach them from the trail?
- What is the pre-existing condition rule?
- What is the start date, and is there a waiting period?
- Are you covered on a restricted-area route with a licensed guide?
- What are the notification and claim deadlines?
- Altitude tier. HGN places the circuit in the ≤5,500 m tier, above Larkya La. Plan selection is based on the highest point of the route, and a Special Route plan applies to designated peaks.
- AMS. Qualifying AMS treatment is included from the ≤5,500 m tier, and qualifying AMS can trigger emergency medical transportation.
- Emergency transportation. Ground, ambulance, helicopter or another suitable method, chosen from medical condition, location, weather and feasibility. It is not a guarantee that every incident results in a helicopter.
- Optional satellite device for positioning and SOS. Confirm availability when you buy.
- Buying in Nepal. Allowed if your trek hasn't started, with coverage from a future date you choose.
- Claims. Report within 48 hours, submit documents within 21 days.
| Benefit | Limit |
|---|---|
| Accidental death & disability | $35,000 |
| Accidental & AMS medical treatment | $3,500 |
| Emergency medical transportation | $4,500 |
| Repatriation of corpse | $2,000 |
Compare the emergency-transportation limit with the possible cost of evacuation from where you would be. CTG focuses on accident, AMS and evacuation. Many travellers hold a separate international policy for cancellation, baggage and broader medical needs. See CTG's Manaslu Circuit plan page, the rescue process, or learn more about Comprehensive Tourism Guard (CTG).

FAQ Section
Is travel insurance required for the Manaslu Circuit? We found no official notice requiring every Nepal trekker to hold insurance. But the Department of Immigration lists insurance documents among those agencies submit for restricted-area permits, and licensed agencies commonly ask for proof of cover. Treat insurance as a practical requirement and confirm the current position with your agency.
What altitude should Manaslu insurance cover? At least the highest point of your itinerary. For the standard circuit that is Larkya La, published at about 5,106 m and by some sources up to 5,160 m, plus any side trips. Choose a band that clears it with margin, such as 5,500 m or 6,000 m, and confirm the band applies to trekking.
Does insurance cover altitude sickness on Manaslu? Some policies do, but it is never automatic. Check that trekking is covered at your altitude, that AMS, HAPE and HACE are named or clearly included, and whether the policy pays for treatment, evacuation or both. Insurers may ask for medical evidence such as oximeter readings when assessing claims.
Does insurance cover helicopter evacuation? Only if the policy includes emergency evacuation, and even then a helicopter is used only when medically necessary, authorised and operationally feasible. Weather, daylight, terrain and landing conditions can prevent flights, and ground transport may be the appropriate method. No provider can guarantee a helicopter at a given place and time.
Is 5,000m coverage enough for Manaslu? No. Every published figure for Larkya La (roughly 5,106–5,160 m) is above 5,000 m, so a 5,000 m limit would not clear the pass. Look for a band of 5,500 m or higher and confirm the wording applies to trekking rather than only to lower-altitude hiking.
Can I buy trekking insurance after arriving in Nepal? It depends on the insurer. Some sell to travellers already in Nepal if the trek hasn't started, with coverage beginning on a future date and never retroactively. CTG works this way. Others impose waiting periods or require purchase before you leave home. Buy before you travel where you can.
What happens if bad weather prevents helicopter evacuation? The response shifts to other options: stabilising the patient, descending on foot with support where safe, moving to a nearby village or health post, and waiting for a flying window. Helicopters fly under Visual Flight Rules, so cloud can delay them. Descent remains the definitive treatment for severe altitude illness.
Does being in a restricted area affect insurance? Restricted status alone does not cancel cover, but policies can require a legally permitted route and correct permits, and some limit cover when you travel against official advisories. CTG's terms require legally permitted routes. Confirm route, permit and closure conditions in your policy wording.
Do I need to carry proof of insurance on the Manaslu Circuit? NTB says your trekking permit must be kept with you and shown on request. We found no rule setting an insurance-card requirement on the trail. Practically, carry your certificate, policy number and the insurer's 24-hour number on paper and offline, and give your guide and agency a copy.
Does standard travel insurance cover the Manaslu Circuit? Sometimes. Some standard policies cap altitude or exclude trekking or hazardous activities, while others cover trekking to higher bands. Compare the policy schedule against Larkya La's altitude and your activity definition rather than assuming either way.
